Amy is a case manager with a professional background in social work. She is a registered professional with several years’ experience leading and managing complex care needs.  Amy had a lead role in her district for discharge planning and management from the regional neurorehabilitation unit at Walkergate Park. 

Amy has worked as both a locality  and medical discharge social worker and is proficient in assessment, reporting writing, discharge planning and care planning. 

​Amy is a skilled case manager who  works effectively in gathering and analysing information from a range of sources to formulate tailored interventions that respond to clients’ needs whilst interpreting and implementing relevant legislation. Amy is passionate about person centred care provision.
