​Detailed analysis, collaborative goal setting and planning leads into a responsive service which supports, coordinates and develops the plan for each client, working with families, services and suppliers toward meaningful and sustainable outcomes, demonstrated by clear robust reporting and feedback.

We provide comprehensive case management or specific short-term assessment and coordination for a wide range of complex conditions across the age-range, including:

​Social Return is a growing case management company covering the North of England. We currently operate a North East Team and a Yorkshire and North West Team and our head office is situated in Durham, near to the city centre. ​

As case managers we coordinate services and support people who have survived serious life-changing injuries and who are rebuilding their lives and futures.

​We employ health and social care professionals who work face-to-face with our clients and their families across the North. Case managers work flexibly at home, in a wide variety of community settings and at our offices where there is always a vibrant and busy atmosphere.

Our North East Case Managers provide services to Client’s in  Northumberland, Tyne and Wear, Teesside and County Durham.

Our Yorkshire and North West Case Managers provide services throughout Yorkshire and the Humber and the North West.

LONDON
We now provide Case Management Services in London and we are planning to add to our team here in the near future.

We may be able to accept client referrals from further afield in some circumstances or suggest an alternative provider so do feel free to contact us for a no-obligation discussion on any case you feel our service may be the right fit, but also if our advice would be helpful.

HEAD OFFICE
​Our Management, Administrative and North East Case Management Teams are based in Durham City, with plentiful on-site parking and wheelchair access. ​

Care Quality Commission (CQC) Information
​

Social Return Case Management can support client's who require care and support via directly employed Support Workers. We are registered with the CQC to provide the regulated activity of Personal Care. We also provide HR and Employment Support and Support Worker Training and Induction Packages, which include delivery of the Care Certificate.
