Katie came into case management as a natural progression from managing and coordinating cases during her career as a social worker. She is experienced in assessing, leading and managing complex care needs across a range of settings. She has extensive experience in discharge planning and case management of transitions for service users with significant and complex needs. She is experienced in report writing, care planning, and providing statements and her records are insightful, clear and comprehensive.
