Traci-Ann is a case manager specialising in clinical and vocational rehabilitation as well as a specialist occupational therapist. Her particular areas of interest are acquired/traumatic brain injury; spinal injury; catastrophic injury; polytrauma; prolonged disorders of consciousness; functional neurological disorder (FND); plus cognitive and functional rehabilitation. Her current caseload covers adolescents and working age adults. Traci-Ann’s extensive experience in managing complex cases includes coordinating and leading large multi-disciplinary teams of both private, statutory and education providers. She works closely with financial deputies regarding clients’ financial requests and budgeting, setting up and running independent living trials (ILTs), managing issues around mental capacity and consent, overseeing care support teams, supporting complex clients to work towards reducing their care support whilst increasing their independence. Traci-Ann encourages client and family involvement in the rehabilitation and case management process by building a strong rapport and skilfully manages expectations of family, their support network and professionals on the case ensuring a collaborative approach to achieving the client’s goals.
