Katie is a registered social worker with several years’ experience assessing, leading and managing complex care needs in community and hospital settings.  She has had extensive experience in discharge planning and case management for service users with significant and complex needs.  Katie has worked as both a locality and medical discharge social worker and is skilled in assessment of needs, reporting writing, discharge planning and care planning, as well as case coordination.  

​Katie works effectively in gathering and analysing information from a range of sources to formulate tailored interventions that respond to clients’ needs whilst interpreting and implementing relevant legislation.

​She is particularly passionate about empowering individuals to achieve their goals, ensuring their rights and wishes are upheld, and helping them to move forward in their rehabilitation.
