Sam is an experienced case manager working with injured children, young people and adults. She has significant experience across a wide variety of clients with neurological and other complex health and social care needs. Sam has experience of multidisciplinary working with a range of professionals and she has provided specialist clinical reports to inform the rehabilitation process.
Complexity and challenge are at the heart of Sam’s caseload and she is able to quickly build rapport with her clients and families and notably wins a high degree of trust from parents of her younger clients. Her calm reasoned approach is valued across the teams she works within. Sam has a deep understanding of safe supported discharge from hospital and transitions between care settings and independent community living. She advocates for clients’ holistic needs and rehabilitation goals in areas such as their care and support requirements, accommodation, equipment and access to the community, liaising with health and social care agencies to coordinate support. Sam’s work as an occupational therapist before she became a case manager gives her a depth of knowledge of what the statutory sector can provide. In this role she covered both hospital and community settings, including as a specialist neurological occupational therapist within a rehabilitation environment.
